Continuing Healthcare

NHS Continuing Healthcare (often referred to as NHS CHC or just CHC) is a package of ongoing care that is arranged and funded solely by the NHS specifically for individuals who have ongoing healthcare needs that have arisen as a result of disability, accident or illness.

NHS Continuing Healthcare is free and it is the responsibility of Shropshire, Telford and Wrekin Clinical Commissioning Group (CCG) to decide the appropriate package of support for eligible individuals.

The attached document is for people who may be in need of ongoing care and support from health and social care professionals and explains the process used to determine whether someone is eligible for NHS Continuing Healthcare.
